DISCLOSURE TO CONSUMER REGARDING BACKGROUND SCREENING

In connection with your employment or application for employment and/or during the course of your employment or in conjunction with your contract work or temporary assignment (which should not be interpreted as employment), The Hotel Group (the "Company") may obtain from a consumer reporting agency, a consumer report or an investigative consumer report (commonly referred to as "background reports").

Background reports requested by the Company may include your background, employment history, academic and/or professional credentials, military service, credit history, and driving history. The information gathered also may involve a criminal history and/or alcohol or drug use history, if any. These background reports may include information about your character, general reputation, personal characteristics and mode of living that may be obtained by interviews with individuals with whom you are acquainted or who may have knowledge concerning any such items of information. This also may include contacts of all listed prior employers to verify your employment history.

Under the provisions of the Fair Credit Reporting Act ("FCRA"), 15 U.S.C. § 1681 et seq.; and certain state laws, before the Company can seek such reports, where permitted, you must provide your written permission to obtain the information. You have the right, upon written request, to a complete and accurate disclosure of the nature and scope of the investigation. You also are entitled to a copy of that document entitled Summary of Rights Under the Fair Credit Reporting Act and it is provided below. Under the FCRA, before the Company can take adverse action on the basis, in whole or in part, of information in a consumer report, you will be provided a copy of that report, the name, address, and telephone number of the consumer reporting agency, and a summary of your rights under the FCRA.

AUTHORIZATION AND RELEASE TO OBTAIN INFORMATION

Under the Fair Credit Reporting Act ("FCRA"), 15 U.S.C. § 1681 et seq., and all other applicable federal, state, and local laws, I hereby authorize and permit The Hotel Group to obtain information, where permitted, pertaining to my employment records, driving history records, driving performance and safety history, criminal history, credit history, civil records, workers’ compensation (post-offer only), alcohol and drug testing, verification of my academic and/or professional credentials, and information and/or copies of documents from any military service records. I understand that an "investigative consumer report" may result that could include information as to my character, general reputation, personal characteristics, and mode of living that may be obtained by interviews with individuals with whom I am acquainted or who may have knowledge concerning any such items of information.

By signing below, I acknowledge receipt of the A SUMMARY OF YOUR RIGHTS UNDER THE FAIR CREDIT REPORTING ACT, and certify that I have read and understand these documents. I hereby authorize the obtaining of "consumer reports" and/or "investigative consumer reports" by the Company at any time after receipt of this authorization and throughout my employment, if applicable. To this end, I hereby authorize, without reservation, any law enforcement agency, administrator, state or federal agency, institution, school or university (public or private), information service bureau, employer, or insurance company to furnish any and all background information requested by Predictive Leadership Solutions, LLC., another outside organization acting on behalf of the Company, and/or the Company itself. I agree that a facsimile ("fax"), electronic or photographic copy of this Authorization shall be as valid as the original.

I further understand that under the FCRA, I may request a copy of any consumer report from the consumer reporting agency that compiled the report, after I have provided proper identification. Where permitted, this authorization shall remain in effect over the course of my employment and reports may be ordered periodically during the course of my employment.

ADDITIONAL STATE SPECIFIC DISCLOSURE

WASHINGTON STATE: If the Company requests an investigative consumer report, you have the right, upon written request made to the Company within a reasonable period of time after your receipt of this disclosure, to receive from the Company a complete and accurate disclosure of the nature and scope of the investigation requested by the Company. 48TYou are entitled to this disclosure within 5 days after the date your request is received or the Company ordered the report, whichever is later. 48TYou also have the right to request a written summary of your rights and remedies under the Washington Fair Credit Reporting Act.

Information Needed to Complete the Background Report:

This information is being collected only for the use of conducting the background check and will not be taken into consideration in any employment decision and will not be used for any other purposes.
