LTYA Basketball Sponsorship/Donation Form

The LTYA Basketball program offers recreational and competitive basketball programs for kids in kindergarten through grade 8, and we need your support. LTYA Basketball is 100% self-funding and receives no financial support from the city, county, or state. With your generous support, our program can continue to grow and provide more opportunities for the kids in our area.

LTYSA offers the following sponsorship/donation opportunities:

Individual Donation $25

Family Sponsorship $100
Your $100 family donation will be recognized on the Basketball page of the LTYA website (www.ltya.org)

Business Sponsorship $400
Your $400 sponsorship will be acknowledged as follows:
Your business name will appear on the back of your team's jerseys
Your business logo and URL will be posted on the Basketball page of the LTYA website (www.ltya.org)
You will receive a team plaque and picture
