* IN CONSIDERATION of our acceptance of your child`s application to play soccer with Vancouver FC ("VFC") and VFC’s agreement to allow your child to play soccer with VFC, I on my own behalf and on behalf of my child, hereby release and discharge VFC and its directors, officers, employees, servants, and agents, including all volunteers who may assist VFC from time to time, of and from all actions, causes of actions, claims, demands, costs and liabilities of every nature or kind whether arising at common law or in equity, by contract, by tort or under any statute or otherwise in any way related to or connected with any personal injury or injuries which my child may suffer as a result of his or her playing soccer with VFC. This waiver shall inure to the benefit of VFC’s successors and assigns. I have read and fully understand all the terms and conditions of this waiver. I was given sufficient time to review the terms of this waiver and to seek independent legal advice before executing this document.

* Vancouver FC relies on volunteers to carry out many of the administrative functions in our Club. We ask all of our families to get involved to help build our Club and enhance your experience with our Programs. Please indicate on the list below at least three areas you are interested in helping with:

Sponsorship Information

If you are interested in sponsoring a Vancouver FC team please include the name of business/organization, contact person's name, phone and email address and our sponsorship coordinator will get in touch.

Registration Completion

In order to complete your registration all players must sign a "Player Code of Conduct" and all parents must sign a "Parent Code of Conduct." Copies are available on our website at www.vancouverfc.info or from your Team Manager. Please submit your signed code of conducts, registration fees, equipment/uniform fees (if applicable), and coaching fees (if applicable), to your team manager prior to July 30, 2011.

Please, payments must be received by July 30, 2010.  This is critically important! Payments received after this date may result in your player not been registered and if this happens they will be unable to play in the season opening games.

Vancouver FC does not wish any player to be unable to participate due to financial hardship. If you need to discuss payment options or assistance please contact your Team Manager.
