APPLICANT'S CERTIFICATION

I certify that the information contained in this application is correct to the best of my knowledge and understand that falsification of the information is a ground for dismissal.  I authorize the references listed above to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release all parties from all liability for any damage that may result from furnishing same to you.  I agree to conform to the rules and regulations of the Company.  I understand that if an offer of employment is exteded that it is conditional upon completeing the Federal I-9 form and providing documents establishing identity and work authorization.  I understand that this employment application and any other company documents are no promise of employment.  I understand that my employment can be terminated with or without cause and with or without notice, at any time, at the option of either the company or myself.  I understand that no manager or representative of the Company, other than the president, has the authority to enter into any agreement for employment for any specified period of time, or make any agreement contrary to the foregoing.

I understand that "Sewell Ford, Inc" may obtain a 'consumer report' including but not limited to your personal background and credit information through credit agencies, records search, or other sources for purposes related to your application for employment or continued employment with Sewell Ford, Inc., including hiring, promotion, reassignment, or retention.  Sewell Ford, Inc. may also obtain an 'investigative consumer report,' as defined by the Fair Credit Reporting Act, to evaluate your personal background including character, general reputation, personal characteristics, and mode of living through personal interviews with neighbors, friends, associates, or other sources for purposes related to your application for employment or continued employment with Sewell Ford, Inc.  The Fair Credit Reporting Act requires that this disclosure inform you of your rights which are disclosed in Attachment A hereto titled 'A Summary of Your Rights Under the Fair Credit Reporting Act.'

AUTHORIZATION TO OBTAIN CONSUMER REPORT AND INVESTIGATIVE CONSUMER REPORT

The undersigned authorizes Sewell Ford, Inc. to obtain background and credit information through credit agencies, records search, or other sources for purposes related to the undersigned's application for imployment or continued employment with Sewell Ford, Inc., including hiring, promotion, reassignment, or retention. The undersigned also authorizes Sewell Ford, Inc. to obtain an 'investigative consumer report,' as defined by the Fair Credit Reporting Act, to evaluate your personal background including character, general reputation, personal characteristics, and mode of living through personal interviews with neighbors, friends, associates, or other sources for purposes related to your application for employment or continued employment with Sewell Ford, Inc. This authorization shall automatically remain effective throughout the application process and employment relationship, should not exist.

Date of birth information below will be used by the consumer reporting agency to try to insure an accrate investigation.  It will not be used in any employment decision.  The Age Descrimination Employment Act prohibits descrimination against persons 40 years of age or older.
