2009 Carl's Jr. Golf Classic Foursome Registration

Please complete the information for each Player or Foursome. Please submit this form by by April 20, 2009. The Tournament will be held on May 5, 2009. The shirts are ordered prior to knowing the golfers, so shirt size is not required in advance. Please call or email Tara at (866) 655-4900, tara@starfran.com, with any questions or changes.

Will you be attending the welcome reception 6-8 pm Monday Night at McCormick and Schmicks? If so, how many? (Reception is included for registered golfers. Guest tickets may be added at $40 each)

The Welcome Reception is a 2 Hour Cocktail and Hors d'oeuvres Party. Dinner Reservations are available in a private room following the reception (seating around 7:30pm). If you would like to reserve a table at the no-host dinner portion, please indicate the number of people and person's name the reservation should be under.
