FIELD REQUEST FORM

In order to streamline the process of making field requests FC ELK GROVE would like to utilize technology in making field requests through the club. Please fill out the information below and it will be sent to the fields coordinator for processing. You should have a response within 24-48 hours.

If referees are requested please be prepared to pay for them at the field before your scrimmage starts. Referees for scrimmages will not be paid by the league.

Costs are:

U9 – U11 = $20.00 per referee

U12 – U14 = $25.00 per referee

U15 – U16 = $30.00 per referee

U17 – U19 = $35.00 per referee
