Bank Account details and National Insurance numbers if you have them (not required for freelance staff).
A picture of your face! A photo for your staff pass. Cropped to head and shoulders. Label it clearly as: NAME_IDPhoto. This needs to be clearly identifiable as you.
Please ensure you have all the details you require - delays to getting this information may result in YOU NOT GETTING PAID ON TIME.
You will be sent a contract for signing that will also need to be returned, but this form helps us to contract you correctly.
