Apply to the 2023 ISNHCP Continuing Education Training Program

When you have finished, click on the "Submit" button at the bottom of this form to send it to the ISNHCP.

If you are unable to finish in one sitting, you can also click on the "Save Progress" button at the bottom of the form. You will be instructed to open up an account. From there you will return to the application and be able to use the "Save Progress" option as often as you like and your current entries will be saved. You can also return to the application at a later time to finish it. [Arming your computer with a surge protector with back-up in the event of a power outage is also recommended.]

Near the bottom of this application is the ISNHCP Oath of Allegiance. Please read this. All ISNHCP students, graduates, and instructors are resposible to uphold and abide by its tenets. Thank you!

After your completed application form and fee have been received and your application has been reviewed, you should receive a welcome email with further instructions within 24 to 48 hours.

There is a nonrefundable Application fee of $200 (USD). Applications will not be processed until payment is received by the ISNHCP.
