Permission and Terms

In consideration for Bishop Chatard High School (BCHS) and its affiliates who are accepting the registrant for this academy, I release, discharge and otherwise indemnify BCHS, its affiliated staff and sponsors, their employees and associated personnel, including the Archdiocese of Indianapolis against any claims by or on behalf of the registrant as a result of the registrant's participation. I also authorize the BCHS personnel to act for me according to their best judgment in any situation that may require medical attention.
