Photo Release: I give permission for my child's photo and photo of her work to be used for St. Mary's and Created for a Purpose promotional purposes, as deemed useful by the camp director. Names of children will not be released. *

It is my understanding that in the event of an emergency involving my child, every attempt will be made to reach me or the Emergency Contact Person I have listed for my child. If I cannot be contacted, I give my permission for camp personnel to provide medical services that are deemed necessary.

Payment/Refund Policy: *

It is my understanding that tuition is due at time of registration. Camper tuition is $250 which includes a t-shirt, lunch each day, art supplies and materials for 5 days. A $40 registration fee is included in the $250 tuition. Before April 15th, you may receive a refund of $210. NO refunds will be given AFTER April 15th. Camp registrations are non-transferable. Created for a Purpose will adhere to all CDC, State and Local Covid guidelines that are in place when camp takes place.

This registration is for the St. Mary's-on-the-Highlands location in Birmingham. I understand that once my registration is completed, I may NOT switch sessions. I have double-checked and registered my daughter for the correct session.

The next page will prompt you for camp payment. Your registration is not complete until that payment is processed. You will receive an email to the address you provided in your registration when your registration is compete.
