(No C4C camp week of July 10th)
Hours: 8AM - 4PM
 
C4C Junior Camp [SK to Grade 3] will be attending camp at:
Virgin Mary's & St. Athanasius Church [VMSA]/CCC - 1245 Eglinton Ave. W.
 
C4C Camp [Grade 4 to Grade 6] will be attending camp at:
CCC2/PCC - 6341 Mississauga Rd.
 
Cost for ONE week is: [$150] per Child, per Week
Mandatory Camp Shirt Cost: [+$10]
 
Add-on costs:
  • Before Camp Service (7AM to 8AM): [+$10] per week
  • Additional Camp Shirt: [+$10] per Shirt

Waiver

I the parent/guardian of the participant(s):

  1. Give permission to my child(ren) [the "participant"], to participate in the Children 4 Christ [C4C] Camp (the "Camp"), a camp authorized by the Church of the Virgin Mary and Saint Athanasius (the "Church");
  2. While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at the the camp.  I understand that I am exposing my Child to inherent risks and hazards. I accept all these risks and hazards and agree that by allowing my Child to participate in those activities and acknowledge that I will be responsible for any injury or other loss which may occur during my Child’s participation of these activities;
  3. Agree that my child(ren), will abide by all the rules and regulations of the Activity leader(s), otherwise he/she will be returned at my expense;
  4. Understand that the camp, involves spiritual activities where dogma, following that of the beliefs held by the Coptic Orthodox Church of Alexandria and its Diocese in Canada, is taught to and shared by the Activity leader(s) with the participants;
  5. I undertake and agree to indemnify and hold blameless the Camp, the Diocese, its Activity leader(s), its members, directors, officers, its contractors and its employees from liability for any claims by me or any third party from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of the Camp, as well as of any medical treatment authorized by the supervising individuals representing the Camp;
  6. Permit the Church, the Activity leader(s), or whom they may designate, to act on my behalf in case of medical emergencies or accidents pertaining to my child(ren), the paricipant, during the Camp;
  7. Authorize the medical doctor, hospital or medical center to act as they see fit to treat my child(ren), the participant, in case of emergencies or accidents; I will be responsible for the cost of the treatment or procedures; and
  8. I hereby authorize the Camp and/or a nurse, instructor, administrator, or other employee (collectively, the “Staff”) to provide this participant with routine first aid and medication as required and within their training.
  9. I permit the Camp, the activity leader(s), or whom they may designate, to act on my behalf in case of medical emergencies or accidents pertaining to my child(ren), the participant(s), during the Camp;
  10. The medical information I have provided is correct and complete, in every respect, to the best of my knowledge. I agree to notify the Camp if any of the above information should change, to my knowledge, after the date hereof, or if I should discover that any information is incorrect. I agree to notify the Camp as soon as reasonably possible, of any medical development in the health of the participant, including signs or symptoms of COVID-19 or exposure to such, knowledge of which would be useful to the Camp in the running of its programs, and interest of the participant.
  11. I understand that I may be required to pick up this participant, or make immediate pick-up arrangements, should the participant exhibit signs of illness that may be indicative of COVID-19.  In this case, return-to-camp policies recommended by public health at that time must be followed.  I further understand and agree that, in the event of an emergency, the Camp may provide, administer, obtain and/or authorize medical care for the participant. I understand that in the event of a serious medical problem or emergency, the Camp will make best efforts to contact me as soon as possible. I understand and agree that this authorization shall remain in effect for the time of the program at the Camp. I hereby release and indemnify the Camp from any and all claims for damages arising from any injury and/or illness to the participant as a result of any accident, illness exposure, or for any other reason arising from participation in the Camp activities.
  12. I hereby acknowledge that any medical treatment will be performed in the Province of Ontario, and that the courts of the Province of Ontario shall have exclusive and preferential jurisdiction to entertain any complaint, demand, claim, proceeding or cause of action arising out of the medical treatment. I hereby agree that if I commence any such legal proceedings, I will do so only in the Province of Ontario, and hereby irrevocably submit to the exclusive and preferential jurisdiction of the courts of the Province of Ontario.
  13. I hereby agree that the resolution of any and all disputes relating to or arising from this agreement and/or the interpretation thereof shall be governed by and construed in accordance with the laws of Ontario and the laws of Canada applicable therein.
  14. Release the Church, its priests, directors, members, Staff, and the Activity leader(s) from all liabilities or responsibilities that may arise from accidents or other events during the Activity.
