Booking information

Dear Parent/ Guardian

If you wish your child to take part in a Bikeability cycle training course, this form must be completed to book a place.
 
CHILDREN MUST BRING THEIR OWN BIKES AND HELMETS.
Course Times
 
Learn to Ride/Beginners; 10-11.00, 11.00 - 12.00 or 12 - 1.00 (These courses last for two consecutive days for children in all Primary years ).
 
Level 1 & 2 09:30 - 14:00
30 minute supervised Lunch (Lunch is not supplied). Level 2 course lasts two consecutive days and the trainee must attend both to achieve a Bikeability certificate and badge. For children in School Year 6 and above, that are comfortable riding a bike with one hand. The course is an introduction to cycling on quiet roads.

2/ Dundonald Primary school, Dundonald Rd, SW19 3QH

I agree that:

I will supply a roadworthy bicycle or adapted cycle for my child or indicate i wish to borrow one.

I will tell Cycling Instructor Ltd about any medical or behavioural conditions my child has that may affect the training course.

Taking a cycling lesson does not mean that it is safe for my child to cycle or fix a bicycle in all circumstances. To become a proficient cyclist or mechanic takes more practice than lessons of this kind can provide.

Please note that in order to ensure that helmets are adjusted properly or to prevent injury it may be necessary for instructors to physical interact with trainees.

I should think about personal accident insurance for my child.

Cycling Instructor Ltd is not responsible for any injury, loss or damage, which may occur during the journey to or from these courses.

My child may be photographed during this course and the image could be used in publicity for Cycling Instructor Ltd and the funding organisation.

Training takes place on the road.

Collection of Information

Cycling Instructor Ltd only holds data, which you have entered into a request form for a cycle lesson.

Use of Information Collected

We will only use the information you provide us for the following purpose:

  • Contact you to provide the lesson
  • We only provide statistical data to the Council and TfL who pay for this service. We do not send any personal data to these organisations.
  • For adult lessons your email will be put on our email system to inform you of any events which may be off interest. You can opt-out if you receive an email.
  • We will hold your data for a year when we will redact it.
