Girl Scouts of Northern California wants to hear how your event went! Please use this form to report your actual attendance, final income/expenses and event evaluation summary. This form should be submitted two weeks after your event.

Make sure to include the reference number you received when you submitted your Event Notification and Approval Form. You may have also received this 8-digit reference number in an email from GSNorCal.
