Certificate of Insurance Request Form

This form is used if a vendor or site you are working with requires a Certificate of Insurance from Girl Scouts of Northern California. If you are reserving a space in your child's school using an online facilities use reservation system, such as (i.e. Civic Permits and Facilitron), read the document, called Meeting Place Considerations in the Volunteer Essentials for further instructions. This is not the form used to request certificates of insurance for those online reservation platforms.
 
Certificates are EMAILED to the person requesting it, and should be forwarded to your facility or vendor upon receival.
