All registration forms are processed on a first come, first served basis.
Please register and pay early as space is VERY limited and sold out in a single day in 2021.

Payment must be made to secure your child(ren)’s place in the program.

Registered participants will receive a confirmation email from the HTY Drama Education Department
as soon as their REGISTRATION FORM AND PAYMENT have been processed.

SECTION 2: SPECIAL CONSIDERATIONS

In order to ensure your child(ren) fully benefit from the HTY program, it is imperative that the instructors are aware of any special circumstances concerning your child(ren). Below please list any information that would be helpful for us to know. Include allergies, recent injuries, physical disabilities, learning and/or behavior issues, and difficult family situations. This information will be kept confidential. If there is nothing to list for your child(ren), please put NONE.


SECTION 3: PROGRAM GUIDELINES FOR PARENTS AND GUARDIANS

Please read and mark the checkbox for the following information as this section will answer many of your questions about HTY’s Summer Program.

PROGRAMS
Both programs focus on the children’s creativity. There are no auditions or casting of roles. Instead, students are challenged to create and develop their stories and characters, dialogue and blocking. In addition to designing and constructing their own set, prop and costume pieces. *
SCHEDULE

Classes are held Monday through Friday from 8:30 am-12:30pm, beginning on Monday, June 6, 2022. The last class will be held on Friday, July 1, 2022.  Class schedule and format is subject to change based on health and safety guidelines.  

Details regarding a showcase of the participant’s work will also be determined closer to the end date pending health and safety guidelines.

ATTENDANCE
Attendance and promptness are important to your child(ren)’s learning experience and participation.  Please give advance notice of any planned absences in writing.  If attendance becomes a serious issue, the student may be dismissed from the program without a refund. However, due to the seriousness of the current pandemic, students who miss class due to illness in their household will not be penalized.  We ask you not plan vacation time during the program. If you have concerns please see the Program Director as soon as possible. *
SUBSTANCE ABUSE
Honolulu Theatre for Youth is a Drug Free Workplace. It is illegal to possess, distribute, or use drugs or alcohol at any of our program sites. If any participant (or parent, guardian or other designated caretaker) violates this agreement, the student will be immediately dismissed from the program without a refund. Please sign below that you have read and understand this. *

SECTION 4: CORONAVIRUS PANDEMIC INFORMED CONSENT AND WAIVER

EXPECTATIONS FOR PARENTS & GUARDIANS
• Share program information with your child(ren) before the first day (see below)
• Be sure to send your child(ren) with their own masks, snacks and water
• Keep your child(ren) home if they, or anyone in your household, is sick or if they might have had contact with someone who is or has been sick (see below for expectations regarding illness outside of class).

EXPECTATIONS FOR PARTICIPANTS
• If you don’t feel well at home, let your parent or guardian know
• If you don’t feel well during class, let the HTY staff know
• Wear your face mask during class time
• As possible keep 6 ft of space between other participants and yourself – HTY staff will give reminders
• Practice good hygiene – HTY staff will give reminders too
• Do not share snacks or drinks with other participants.

ILLNESS DURING CLASS
• If a participant feels ill during class, staff will move them to an area away from other participants while they wait to go home.
• In the following week, staff will follow up to see how the participant is feeling and assess if other families need to be informed of possible illness.
- HTY staff request that if the illness is determined to be COVID-19, parents should keep the child at home and alert HTY immediately. HTY will follow-up with other parents.

ILLNESS OUTSIDE OF CLASS
If a child, or member of the child’s family, does become ill, HTY staff requests the following:
• If a cold or flu, parents should keep the child at home.
• If the illness is determined to be COVID-19, parents should keep the child at home and alert HTY staff immediately. HTY will follow-up with other parents.

SECTION 5: AUTHORIZATION

By checking the authorization box below, I, (designated family spokesperson/representative) choose to enroll my child(ren) in an HTY summer program.

I understand there are inherent risks associated with drama activities and potential exposure to the Coronavirus, and I accept all known dangers and risks. I authorize HTY or any of its employees to refer my child(ren), if injured or ill, to our family physician when I or my child(ren)’s alternate guardian cannot be reached. If no family physician is designated, HTY or any of its employees are authorized to select a physician when deemed necessary.
 
I authorize and give full consent to HTY to copyright and publish any photographs and/or video recordings in which my child(ren) appears while participating in this program. These photographs and/or video recordings may be used for any and all promotional and educational materials related to HTY, and I agree to their use for that purpose.

I understand that prolonged absence or tardiness will affect participation in the program (except in the case of serious illness).

I hereby waive any and all rights, claims, or causes of action arising from property damage and bodily injury or any contraction or infection of the COVID-19 virus as a result of my child(ren)’s participation in HTY activities and I hold harmless HTY (including their Board of Directors, employees, members, agents, and representatives). I knowingly and voluntarily agree to this waiver for myself, my heirs, executors, assigns, and personal representatives.

ADDITIONAL REGISTRATION INFORMATION:
 
Tuition: Tuition for our Summer Programs is $600. Payment must be made online, by mail, or by phone to secure your child's place in the program.
 
Sibling Discount: If multiple children from a single household enroll, tuition will be $525 per child. For two children, total of $1,050. For three, total of $1,575.
 
Refund Policy: Full refunds will be given if cancellation is received by Friday, May 27, 2022. Full refunds (less a $100 processing fee) will be given until the first day of the program, Monday, June 6, 2022. No refunds will be given after the first class. Participants withdrawing after the first day of the program will receive tuition credit toward other HTY programs. No refund or tuition credit will be given after the second day of the program.
