PLEASE READ BEFORE COMPLETING THIS FORM

Students: Complete all of your information in the student section on Page 2 of this form. Once you hit the submit button at the end of the form you will find a print option. Print out the form and bring it to your proctor for completion. Completed form must be received 2 weeks prior to the test date.

Proctors: Sign, date, and return the completed form to Alexis Thompson - Mack Library.

Completed form must be received by regular mail, in-person, or by fax 2 weeks prior to the test date. Please allow for normal mail delivery.

This form is to be completed by students taking a Proctored Exam outside of the Monroe or Bethlehem Campus Libraries.

By submitting this form, the proctor agrees and commits to providing:

* A secure proctoring service for the NCC student

* The equipment (computer, Internet access, desk, etc.) to complete both a paper based and/or an online exam

Proctoring Options

Students have the option of arranging proctored exams at an academic testing center. The following link provides examples of such sites thoughout the United States:

Also, students can contact one of the following proprietary testing centers to determine if they can supervise proctored exams:

Military students can also contact their military education officer to schedule their proctored exams.

Many public libraries offer proctoring services as well.
