This form is for all students.  
 
This form is not for OPT enrollments.  Students must email opt@mum.edu to apply for OPT.  
 
This form is not for DE enrollments.  The DE office will hold a meeting for each entry to instruct them as to how to enroll for DE courses.
 
This form is also not for requesting a swap.  
 
Registration change requests must be made by the last day of the block before the course you are requesting.  Requests received over the weekend before the course starts will be denied. 
 
This form can be used to:
A.  Request either a course change (when seats are available) or a wait-list request (if a course is full). 
 
B.  Request a course if you have taken a month off and need to add your last course. 
      (1)  You cannot take more than 1 month off if not going out of the USA.
      (2)  Your I-20 will go into automatic termination if you take a maximum of 5 months out of the USA.
 
C.  Request a course change if you have changed a course and have the same course in another block
  
1.  This form will automatically be forwarded to your advisor who will alert the Registrar to any special reasons you need to be in this course.  So, please contact your advisor if you have a situation you feel makes it important for you to be in the couse and s/he will contact the CS Registrar regarding this request. 
 
2.  Students may request only one course change per block. If two requests are submitted, you will be added to the first course that is available when it is your turn on the waiting list. Therefore, you should use this form for the course you want the most. 
 
3. The best way to get into a course that is full is to swap with another student already in the course.  Here is a link to the MIU mobile app in both Google and Apple stores.  You can use this to notify other students of your interest in switching courses. 
4.  Final course assignments will occur on the weekend before the block begins. 
 
Filling out this form is no guarantee of a seat, but will automatically put you on the waiting list.  PLEASE NOTE:  SUBMITTING THIS FORM DOES NOT ENROLL YOU IN THE COURSE YOU HAVE REQUESTED, IT ADDS YOUR NAME TO THE WAITING LIST IF THERE ARE NO SEATS AVAILABLE.  YOUR REQUEST MAY NOT BE PROCESSED UNTIL THE BREAK BETWEEN BLOCKS, SO PLEASE BE PATIENT.
