The Department of Homeland Security (DHS) requires all F-1 international students who are engaging in Post-Completion Optional Practical Training (OPT) and STEM to report all changes to employment within 10 days of a change.
F-1 students on Post-Completion OPT and OPT STEM are required by law to report employer information each time they accept a new position and must also report termination of employment and all periods of unemployment.
Please use this form to report a new employer, changes to your current employer's information, your last date of employment when you leave a position, and all periods of unemployment.
If you are not an F-1 student engaging in Post-Completion OPT, you should not use this form to update your employment.
