Surfers Paradise Catholic Parish Baptisms Form

If you do not yet have the answer to a particular question below and the question is required, please put in an answer such as TBC (to be confirmed)  or something similar that fits the category. you can update us with further information later.
Please note that Baptism is not a private event.  Baptism is entry into the Family of God's people, becoming a member of God's family in the church.  This sign is best shown by having a few baptisms at the same gathering.  This is not merely a convenience but a symbol of our entry into the community of the church.  Could you please let your family and guests know of this understanding, as we sometimes hear people saying as they enter, "oh, it's not private, there are other people having baptisms"  as if this is a surprise or not a good thing.  We wish for people not to have any surprises and to understand the wonderful celebration of belonging to a family bigger than our own individual family. The actual baptisms are family by family, but the introductory and concluding elements are shared. It works really beautifully. Every family is treated as special. Please indicate that you understand. *
Please note that food, hot drinks, and soft drinks may not be brought into or consumed in the church building at any time. Food or drink should be consumed outside the church before entry.

Whilst inside the church, we ask guests to keep their voices respectfully low as this is a place of reverence and worship of God.

In ordinary circumstances, men should not wear hats or caps inside the church.

Provided as part of the cost of the baptism is a Baptismal Stole. (We will have the stole for you on the day). However, you will need to provide a Baptism Candle for the ceremony. The parish piety stall has a selection of candles you can purchase before the event. (The cost of the candle varies according to size, $4 - $30). The candle and other items are available from the Surfers Paradise Parish Religious Goods’ Shop at Sacred Heart Church. This shop is open via the parish office from Monday to Friday (9 am until midday) and before and after weekend masses. You can also order and design baptismal candles from many online sellers.  *

There is an ancient tradition whereby each Christian Child is given a name of a patron saint as a particular inspiration during their life.   If your child does not already have a Saint's Name in their given name,  please consider choosing an additional name for the child that can be their patron saint name. You can check for various suitable saints' names from this link here -  https://www.catholic.org/saints/stindex.php

If you wish to add a Christian Saint name as a patron to the names you have already chosen, please write the saint's name in the space below.
In relation to the person to be baptised, please select one of the following options. *
PLEASE SELECT ONE OF THE FOLLOWING OPTIONS *
I understand that the family provides a suitable baptismal candle (or the godparents or relatives design or purchase one for them), which they will purchase either from a Christian store or various online sellers and bring along on the day of the Baptism. I also understand that the church here presents you with a baptismal scarf that will be placed around the child's shoulders after the pouring of water to show that this child has been clothed in Christ and his life. *

DETAILS OF GODPARENT(S). PLEASE NOTE THAT YOU NEED AT LEAST ONE GODPARENT, BUT YOU ARE WELCOME TO HAVE TWO OR MORE. ONE MUST BE CATHOLIC. THE OTHERS SHOULD BE CHRISTIAN. According to Church Law, the Godparent must be at least sixteen years old and have received the sacrament of confirmation and First Holy Communion. Certainly at least one Godparent must fulfil these requirements, but preferably all do. If you have any questions about this, please feel free to contact the Parish Office

I understand that I need at least one Godparent. This godparent must be Catholic, at least 16 years of age, and have received their sacrament of confirmation. It would be ideal if that catholic godparent is present at the baptism on the day, but they can be represented by another if they are not able to be present on the day. Preferably all godparents should be Catholic, or at least Christian, but in any case at least one godparent MUST be catholic. *
I understand that before my child's baptism can be confirmed, I will need to provide all the necessary paperwork and certificates required. Particularly the signed (both) parent's consent form, which is a separate document, baptismal certificate of one of the parents, and copy of birth certificate of child to be baptised. *
