Notice of Risk and Media Agreement
Event Registration Form

This Risk and Media Agreement is used for those attending our In-person and Online Events.  You MUST be 18 years or older to sign/submit this form. Attendees under the age of 18 must have their parent or legal guardian sign/submit this form.

FOR IN-PERSON EVENTS: Our in-person events (unless stated otherwise) are hosted at our new facility "The Academy of Creative Arts" at 12 A Street in Burlington, MA
                                   

FOR BIRTHDAYS: If you are attending a Birthday Party, under the Birthday Party Host Name or Event Type, please input the first and last name of your host's name (i.e. the birthday boy or girl's name).

FOR ONLINE DIGITAL ADVENTURES: Your child must have a computer (preferably laptop) with working camera, microphone, speaker, and compatible with Zoom to participate in this event/party

Due to popular demand, we are hosting a new option for our heroes who want to sign up for our D&D events but don't want to worry about filling out a form every month. Head over to our brand new Story School Recurring Event Registration Form. We will keep your card on file and register you for the events you choose (Tue, Thu, or Sun D&D;) every month on the first of the month and you won't have to worry about filling out a form each time. Please note that if you take this option, we will need 30 days notice in order to cancel your event "subscription" if you choose to cancel your subscription.

Physical and Behavioral Accommodations

Participation in our online events require a computer or tablet with audio and video capability, and WiFi connectivity to the internet. You will be sent a link/invitation to this event after this waiver is complete at the email immediately below. You need only click the link shortly before the party is scheduled to begin. We recommend that you connect to the event about 10 minutes prior to the scheduled start time to allow for any technical issues that may arise. I acknowledge that I understand that connectivity issues are the responsibility of the guest and that Story School will not make any refunds due to the failure of your technology. *
Do any of the above named participant require special accommodations or demonstrate any behaviors that could require adjustments to physical activities? IE: Are they easily frightened by masks?  Do they get upset easily if rules are broken?  Do they get over-stimulated in physical activities?  Have they hit things in anger? Do they have coping strategies for stress? Do they have any physical limitations? Do they suffer from epilepsy or any other condition that could make dark and mood lighting an issue? *

All of our programs are presented in English, though we work hard to make our presentations very physical to allow for context clues. For the sake of making our events as streamlined as possible please answer the following question honestly, do any of the above participants have any issues understanding or speaking English? *

All participants must wear clothing that is covering, comfortable, and safe for them to stand and move around while in their own space. *
